When completing the roster please remember the following:
Annual dues are $15 ($10 national
and $5 for state)
Lifetime dues are $150
Do not write in the yellow column
Verify information listed for each member
Make corrections by drawing a line through the item
and print legibly above the item
Draw a line through names of those who are no longer
members
List new members at the end of the roster
If an annual member would like to receive the FFA New
Horizons Magazine include $2.00 and mark that amount
in the magazine column
Lifetime members receive the FFA New Horizons Magazine

To be chartered you must maintain at least 10 members
- if a member signed 15 annual members or 5 lifetime
members please have them fill out the Legion of Merit
Citation application. That form can be found
at the forms link
- an Affiliate Fee was established a few years ago
by the state delegates. The Affiliate Fee is
to help maintain the communication and public relation
needs of the alumni. If you are an established
affiliate please mark “affiliate fee” on
the memo line of your check and submit with your roster
and dues check. This Affiliate Fee was initiated
instead of a member dues increase
